MANY investigators have shown that hemodialysis patients are at high risk for hepatitis C virus (HCV) infection. This illness is usually asymptomatic in these patients, but chronic HCV infection has been associated with several extrahepatic immunologic conditions: essential mixed cryoglobulinemia, porphyria cutanea tarda, membranoproliferative glomerulonephritis, autoimmune thyroiditis, and Sjogren syndrome. Positive serologic tests for anti-nuclear antibodies (ANA), smooth muscle cell antibodies (SMA), and rheumatoid factor (RF) have been found in patients with chronic HCV infection. Interferon-alpha (INF-α) is the one of the important treatment options for HCV infection, although frequent relapse has been observed after therapy. Certain patients with cryoglobulinemia and other similar features are among those who have been treated successfully, however. In this study, we investigated the prevalence of autoantibodies in anti-HCV (+) hemodialysis patients and compared these findings to those in anti-HCV (-) patients. We also studied the importance of HCV-RNA positivity and INF-α treatment relative to the occurrence of autoantibodies.
