The use of polyvalent pseudomonas vaccine in patients with burns is described from New Delhi, India. The vaccines contained extracts of surface antigens from the 16 internationally recognized serotypes. The trial included adults with 15-50% burns and children with 5-30% burns (full skin thickness), admitted within 72 hours of burning. Adults received 1.0 recommended human doses (not specified but based on animal experiment) and children 0.5 to 1.0 recommended human doses subcutaneously on days 0, 7 and 14. Swabs for culture were taken on days 0, 4, 7 and weekly till discharge. On admission only serotype 11 was found in those already colonized. Types 7, 022, 2b and 12 were acquired by some patients during the first week. Type 11 was found in 43% of patients at some stage. Vaccinated subjects acquired pseudomonas more slowly than the control group. The results are interesting. No deaths were noted in 18 vaccinated subjects whereas no less than 8 of the unvaccinated group of 20 died. Subjects had been admitted alternately to the vaccinated and control groups and in fact the vaccinated group had burns slightly larger than those of the control group. [There is no mention of the serological responses following vaccination.]K. C. Watson.
