Investigations were carried out to evaluate the role of secretory immunoglobulin A (11S IgA) in the recurrent infections of chronic bronchitis. The IgA content in sputum was related to damage of the bronchial epithelium and the degree of inflammation using the concentrations of lactic dehydrogenase, deoxyribonucleic acid fibers, and fibrinogen in the sputum as criteria of inflammation. The IgA content in sputum of patients with mild to moderate bronchitis of short duration increased significantly as a result of an inflammation-producing stimulus whereas patients with severe, far advanced bronchitis of long duration failed to show this response. Immunodiffusion studies suggested that in the latter patients the secretory “piece” was absent from the sputum. It is therefore assumed that the production of the secretory “piece” was partially or totally disturbed due to the damage of the bronchial epithelium.
