During initial examinations of 8641 persons representing 88% of the population of a Michigan community, thyroid enlargement was observed in 124 males (2.9%) and 451 females (10.2%). Goiter persists at an excessive frequency, especially among women over age 19 (15%), in spite of measures to correct iodine deficiency which have been in effect for over 30 years. Thus, endemic goiter remains an important problem in preventive medicine. Goiter prevalence in school children does not indicate goiter prevalence in the total population. Iodine prophylaxis has accomplished much in decreasing the prevalence of goiter, but the consumption of iodized salt remains inadequate. In Tecumseh, only 62% of salt purchased for human consumption during one winter month was iodized, a level considerably below the reasonable goal of permanent and general goiter prophylaxis. In this population, persons with goiter did not differ appreciably in height and weight from those without goiter. No definite associationbetweenparity and goiter prevalence couldbe demonstrated. Mean protein-bound iodine levels were determined in random samples of persons age 20 to 79 years of age; values did not differ significantly between the sexes or between individuals with and without goiter. In this initial survey, goiter was not found to coexist within sib ships or in mothers and their children more often than would have been expected by chance alone.
