Clozapine is the only antipsychotic approved by the Food and Drug Administration (FDA) for treatment-resistant schizophrenia (TRS)(Kane et al 1988). It is also approved for suicidality in schizophrenia or schizoaffective disorder, and is used off-label in bipolar disorder, Parkinson’s disease with psychosis and for aggression. Several new reviews and guidelines have appeared recently in this and other journals. These along with recent changes in FDA Clozapine risk evaluation and mitigation strategy (REMS) requirements warrant this update for the practicing clinician. Considerations of clozapine use in the context of the ongoing Coronavirus Disease 2019 (COVID-19) pandemic will also be discussed.
