The paper by Martin-du-Pan and Bischof (1995) raises the whole question of assessment of spermatogenic activity in normal and infertile men. The interpretation of follicle stimulating hormone (FSH) concentrations and the value of testicular biopsies are discussed. In this debate, we wish to take a second look at Sertoli cell function in the human, and explore new parameters which might be used as markers of disturbed cell function.Within the seminiferous tubule of the mammalian testis, the structure of the Sertoli cells and the sophisticated junctions between them and the neighbouring germ cells contribute (together with hormonal supplies) to bring all the nutrients required for complete development of germ cells and, consequently, to maintaining spermatogenesis. In the human testis, the basic organization of the Sertoli cells is similar to that described in the rat and specific junctional complexes have been demonstrated between not only Sertoli cells but also between Sertoli and germ cells, and between Sertoli cells and the extracellular matrix (for reviews see Russell, 1993; Schulze and Holstein, 1993).
