[alpha] 1-Antitrypsin- deficient plasma of two patients with severe pulmonary emphysema and homozygous ari-antitrypsin deficiency was tested for its thrombin- and plasmin-neutralizing capacities. In contrast to normal, defibrinated plasma, [alpha]1-antitrypsin-deficient plasma lacks fast antithrombin clotting activity. The anticoagulant activity which remains is of the slow, progressive variety and constitutes approximately 20-25% of the total antithrombin anticoagulant activity of normal plasma. This finding indicates that [alpha] 1-anti-thrombin is a fast antithrombin which represents at least 75% of this type of anticoagulant activity of blood. It also suggests that it is probably identical with [alpha]1-antitrypsin. In contrast, [alpha]1-antitrypsin-deficient plasma has the same thrombin esterase-inhibiting activity as normal plasma. Fast antiplasmin concentrations, however, were reduced by approximately 50% in [alpha]1-antitrypsin-deficient plasma. These findings are consistent with the concept that [alpha]1-antitrypsin represents an antiproteinase with a wider range of activity than its name would suggest.
