T cell-directed immunosuppression only transiently delays the loss of beta cell function in recent-onset type 1 diabetes. We argue here that the underlying disease process is carried by innate immune reactivity. Inducing a non-polarized functional state of local innate immunity will support regulatory T cell development and b cell proliferation.
