SILVER NITRATE prophylaxis against ophthalmia neonatorum due to Neisseria gonorrhoeae, first used by Crede in 1880," T" is one of the triumphs of preventive medicine. Possibly because of changing epidemiologic circumstances, however, reservations were first voiced over 40 years ago by Lehrfeld,"*" and continued to be expressed by Leff,** Shaw,""," and others into the present decade.The major problems with AgNO,, prophylaxis have been (l) it does not afford perfect protection against gonococcal ophthalmia neonatorum;(2) it causes chemical conjunctivitis;(3) it is difficult to administer;(4) it may cause ocular damage through inadvertent use of strong solutions;(5) it may be superfluous in many settings.
