Loss of integrity of pulmonary parenchyma with the development of multiple, large, aircontaining spaces has been described under such terms as air-containing cysts (1), emphysematous bullae (2), giant cysts (3), and" vanishing lung" syndrome (4). The plethora of nomenclature attests to the confusion regarding pathogenesis, and we shall use the descriptive term" giant bullous disease." Scattered reports of single cases (5-8) of associated giant bullous disease and bronchogenic carcinoma have appeared since 1883, but there has been only one report of a series, that of 10 cases by Korol (9) in 1953. Korol suggested that there was a more than coincidental association of what he termed" congenital" emphysema and bronchogenic carcinoma. In this communication the incidence of giant bullous disease in 411 cases of carcinoma observed over a ten-year period in this hospital is compared with the incidence of giant bullous disease in a randomly selected control series of male patients without carcinoma admitted during the same period. The clinical and roentgenographic features of bronchogenic carcinoma and giant bullous disease as they occurred together in 18 patients are described.
