This study is conccrned with the condition of the minute vessels of the inner car as seen in a collection of temporal bones obtained at autopsy from patients of widely varying ages. Its purpose is to examine the microvascular pathology that occurs with aging and to consider its possible relation to presbycusis. Because of the difficulty of setting up any standards of normal vascularization, the approach is qualitative and descriptive rather than quantitative and statistical.In focusing attention on vascular changes found in the inncr ear postmortem, we can offer no assurance that they always precede the degcneration of cochlear tissues and impairment of hearing as the patient grows older. Conceivably, these are independent manifestations of the process of aging. In most of the cases studied the time course of the hearing loss is entirely unknown, and its final magnitude can only be inferred from the condition of Corti's organ and its nerve supply. Furthermore, the role of factors other than aging, such as noise exposure, ototoxic drugs, and disease, can seldom be assessed. With these limitations in mind, we believe that the observations to be presented are at least suggestive of an important causal relationship between vascular changes and presbycusis.
