The treatment of renal calculi continues to be refined and improved and the great majority of cases are now treated using non-invasive/ minimally invasive techniques. The principal treatment modalities in use today comprise ureterorenoscopy, extracorporeal shock wave lithotripsy (ESWL) and percutaneous lithotripsy, while open surgery remains an option for difficult stone cases, such as large staghorn stones with extensive calyceal in-volvement, and for secondary intervention. This paper discusses the use of these approaches for the treatment of difficult stone cases, noting important considerations for their use and the complications that may arise.
