At a New York Academy of Sciences meeting in October 1990 on “Antigen and Clone Specific Immunoregulation”, much of the basic and clinical research relating directly to photopheresis was presented. The proceedings of that meeting will be published in July of 1991, providing the most updated and detailed review of the subject. This overview will highlight the most salient observations reported there.Photopheresis is a treatment that appears to be efficacious in the management of certain patients with disorders of circulating T cells. Much remains to be learned about its mode of action, about why some patients respond well and others do not and about the best way to administer it. Yet, major progress has been made over the past 8 years on both the clinical and scientific fronts, since the initial chance discovery that photopheresis produced a surprisingly profound response in two of the first five patients with erythrodermic cutaneous T cell lymphoma. The procedure is now operative in more than 50 medical centers in the US and in several hospitals in Europe, Japan and Australia.
