DIURETICS, HYDRALAZINE To start with the four exceptions, there is one report of a cholestatic reaction to c/i/oro/hiuiit/e (Drerup et al. 1958), one of a cholestatic (MSD 1964) and one of a hepatocellular reaction to eihaayric ad (Datey et al. 1967) and one of a granulomatous hepatitis concomitant with an arthropatic febrile reaction to/iy (/ru/-mine (Jori & Peschle 1973). Provocation test was performed only in the third case, but also in the other cases the connexion between the drug and the liver reaction appears rather convincing. All these reactions have been reversible after discontinuance of the medication. They have appeared after a short period of medication with ordinary doses and thus appear to have been hypersensitivity reactions.ALPHA-METHY LDOPA LIVER INJURY INCIDENCE The association of hepatocellular dysfunction with administration of alpha-methyldopa was first recorded by Gillespie in 1960. Among the first 1,500 recipients of the drug, only 1 1 instances of hepatic dysfunction occurred, an incidence of less than I%(Weil et al. 1963). On the other hand, in a survey of studies published up to 1969 (Elkington et al. 19691, an incidence of 6.3% of 823 patients was described. However, the incidence varied enormously (from 0 to 35%) between different investigations. Since provocation tests were not regularly performed in these series, many of the recorded hepatic reactions may not have been induced by alpha-methyldopa.
