Despite modest advances in diagnostic techniques and effectiveness of treatment, Mycobacterium tuberculosis remains one of the pathogens causing the greatest amount of chronic disease and death throughout the world. More than 20% of the world's population is infected with the tubercle bacillus. Every year, there are 8 to 10 million new cases and 3 to 5 million deaths attributed to tuberculosis. 1 Children less than 15 years of age in developing countries have 1.3 million cases and 450,000 deaths annually from tuberculosis. 2 Only recently has the enormous economic impact of this disease been appreciated by the World Health Organization and national governments. 3 After decades of consistently declining incidence, a remarkable resurgence of tuberculosis is occurring in the United States. In 1990, tuberculous disease developed in more than 25,000 adults and children. Between 1987 and 1990 the number of tuberculosis cases among children less than 5 years of age increased 39% in the United States. Historically, tuberculosis case rates have increased when populations have been exposed to stresses such as war, displacement, poor nutrition, and inadequate health care.
