Optimal management of infectious diseases depends on establishing a specific microbiological diagnosis. In some cases this can be accomplished directly by the demonstration of the organism, its genomic frag-ments, antigens, or metabolic products in the tissues or body fluids of the host. Over the past decade there has been considerable interest in applying the techniques of molecular biology, immunology, and bio-chemistry to develop clinically useful diagnostic tests. In other cases, diagnosis depends on the development of a specific host-immune response (usually a rise in serum antibody titer). The goal here has been to identify antigen (s) that can be used to distinguish colonization or past infection from active disease. Pneumocystis carinii typifies organisms that have remained outside the mainstream of modern biotech-nology. Although the clinical importance of P. carinii as an opportunistic pulmonary pathogen has long been appreciated, knowledge of the organism's basic immunobiology has remained at a rudimentary level because of difficulties in obtaining sufficient quantitites of purified organisms for laboratory work. However, with the emergence of AIDS, this situation is beginning to change. The pressing clinical problems posed by the dramatic increase in the number of cases of pneumocystosis in patients with AIDS have stimulated interest in the organism. The recent study by Tanabe et al.[1] using DNA hybridization analysis to diagnose P. carinii is an example of this interest. It seems appropriate to review the present status of methods of diagnosing pneumo-cystosis and suggest directions for possible future research.
