Background‘Body schema’refers to the internal model of physical self boundaries that allows us to differentiate ourselves from the world at large. Body schema deficits are a potential mechanism underpinning cardinal symptoms such as somatic distortions/delusions, abnormal interpersonal interactions (eg affective touch) and impaired self/non-self differentiation in neurodegenerative diseases. However, body schema processing remains poorly defined in patients with dementia, in part because we lack tools to characterise it adequately. We addressed this issue using the paradigm of touch (a core component of the normal body schema) in a cohort of patients with frontotemporal dementia and Alzheimer's disease, referenced to healthy older individuals.MethodsWe created a novel set of tactile stimuli sampling a range of animate and inanimate textures of variable valence. Participants rated the pleasantness of each stimulus on a Likert scale and identified it from a word-picture array in a three-alternative forced choice procedure. Autonomic arousal was assessed simultaneously using pupillometry.ResultsThe patient groups showed syndromic signatures of tactile processing impairment with evidence of dissociated cognitive, affective and autonomic coding of tactile information.ConclusionsIt is feasible to assess tactile cognition and reactivity in patients with dementia and this is a promising paradigm to dissect deficits of body schema processing in different dementia syndromes.
