Physical activity reduces the risk of developing cardiovascular disease, the leading cause of death in the United States (1). Recently, many studies also have explored the association of physical activity with cancer, the second leading cause of death (1). The hypothesis that physical activity protects against cancer development is not new. This hypothesis may first have been tested in 1922, when investigators examined cancer mortality rates among men with different occupations. Differences in these rates were attributed to the variation in occupational physical activity (2, 3). Subsequently, little further work was done until the past three decades, when the number of publications examining this hypothesis has increased steadily (1). In this presentation, we evaluate the hypothesis with reference to prostate cancer. We discuss plausible biologic mechanisms for an inverse association between physical activity and prostate cancer incidence, summarize the epidemiologic literature, and propose some areas for future research.
