A case in a 40-yr-old Hispanic woman is reported. She presented with a 2 week history of fever, pleuritic anterior chest pain, progressive dyspnoea, orthopnoea and pedal oedema. A chest radiograph revealed massive cardiomegaly and pulmonary oedema. An emergency operation was carried out to strip the pericardium.Cryptococcus neoformanswas cultured from the drained pericardial fluid. Cryptococcal antigen in the serum was positive at 1:64. Cerebrospinal fluid was negative for cryptococcal antigen and the blood was sterile. Amphotericin B (total dose 2500 mg) and 5-fluorocytosine therapy was successful.
