A 33-year-old woman was admitted to hospital in the 29th week of pregnancy because of progressive symmetric ascending weakness of her legs and arms and respiratory distress. Guillain-Barré syndrome (GBS) was diagnosed on the basis of clinical signs, cerebrospinal fluid examination, and electrophysiological studies. High-dose intravenous immunoglobulin (0· 4 g per kg of body weight per day for 5 days) did not lead to clinical improvement and plasma exchange was started. Her condition gradually improved over 4 months. There was serological evidence of recent cytomegalovirus (CMV) infection. Serum IgM ganglioside GM1 antibodies were strongly positive, whereas IgG ganglioside GM1, GM2, and GQ1b antibodies were negative. The plasma filtrate's effect on neuromuscular transmission was investigated in mice by recording quantal endplate currents.
