Synovial fluid from the knee joints of 268 normal adult male volunteers (age 21-45 years) was collected, and the joint cavities were washed with physiological saline solution. The hyaluronic acid, protein and sialic acid concentrations and the limiting viscosity numbers were determined in both synovial fluid and joint washings. The hyaluronic acid was separated by electrodeposition, and its molecular parameters calculated from sedimentation, diffusion, viscosity, double refraction of flow, and light-scattering measurements. Three pooled samples of arthritic synovial fluids (85 donors, ages 17-72) were analyzed and used for the preparation of pure hyaluronic acid. The limiting viscosity number, average molecular weight and molecular diameter were found to be significantly smaller than those of the hyaluronic acid prepared from normal joint fluid. The hyaluronic acid, protein and sialic acid content, and limiting viscosity number of hyaluronic acid were determined in 7 individual samples of joint fluids obtained from four patients with rheumatoid arthritis. In rheumatoid arthritis the total hyaluronic acid and protein content of the joint and connected cavities is increased. At the same time, the sialic acid and hexos-amine content of the proteins is also elevated. Rheumatoid synovial fluid and the hyaluronic acid prepared from it exhibit non-Newtonian flow behavior but to a significantly lesser degree than normal synovial fluid. The differences between the rheological characteristics of the normal and rheumatoid arthritis synovial fluid may originate from the differences in the concentration, molecular parameters and molecular interaction of hyaluronic acid.
