The reliability with which we can hope to assess schizophrenia is limited because the diagnostic criteria are based on behavioral signs and symptoms that may be multiply determined and subjective. However, many of the crucial behavioral criteria are signs and not symptoms and, as such, are accessible to objective and quantitative measurement. For example, the affective disturbances of schizophrenia are encoded in the patients' pattern of vocal emphasis, or lack of emphasis, and can be measured through study of vocal acoustics. Such approaches permit us to begin to distinguish between affects, moods, and emotions, aspects of feelings that are differentially disturbed in different disorders. Thought disorder and even hallucinations are accessible to objective approaches, the former through psycholinguistics, the latter through study of perceptual mechanisms. In our beginning attempts to utilize these approaches we are impressed with the central role that language disturbance plays in the cardinal signs of schizophrenia: in thought disorders, in vocal auditory hallucinations, and in flat affect.
