For the first time, morphologic evidence is provided that the human fetus is capable of immunologic response to an infectious agent within the first trimester of gestation. A case report of a spontaneous abortion at 13 weeks' gestation is presented. Outstanding pathologic findings were placental plasma cells and villitis, and numerous cytomegalovirus inclusions, in both the placenta and various organs of the 10-cm crown-rump length male fetus. Immunologic response to cytomegalovirus infection of the fetus is evidenced by the presence of placental plasma cells.
