RCC is the 10th commonest malignancy and has an increasing incidence, accounting for an estimated 95 000 deaths per year worldwide. The prognosis is poor for patients with metastatic disease and response rates to chemotherapy are consistently low. Early casecontrol studies and more recent randomized data suggest that the use of interferon-a is associated with a modest survival benefit at the expense of constitutional toxicity. There are <5700 cases of RCC per annum in the UK, comprising 3% of all malignancies [1]. Localized RCC can often be cured by surgery [2]. Despite apparently localized disease at presentation, 20–30% of patients relapse after radical nephrectomy, usually with distant metastases, and a further 20–30% of patients present with metastatic disease [3]. These patients have a 5-year survival of 4% and a median survival of 6 months [4]. Metastatic RCC is often highly variable in its clinical course, including spontaneous regressions [5]. The challenge has been to develop treatments with improved efficacy and in particular to achieve this with minimal side-effects if possible in the outpatient setting. The most promising systemic therapy for metastatic RCC is immuno-(biological) therapy. Experiments reported during the 1960s and 1970s showed that the interferons had antitumour activity in a wide range of laboratory models. Their exact mechanism of action remains in debate although they appear to exert their effect by: (i) stimulating phagocytosis by macrophages; (ii) up-regulating MHC class I molecules; and (iii) having a directly cytotoxic effect on tumour cells [6]. The results of early unrandomized studies suggested that biological therapy with interferon-a produced an objective tumour response rate of < 15% in selected patients that translated into a survival benefit [5,7].
