The respiratory distress syndrome (RDS) of shock and trauma defines itself. It goes by many other names, including shock lung, post-traumatic pulmonary insufficiency, wet lung, daNang lung, adult hyaline membrane disease and haemorrhagic lung syndrome. We prefer the term RDS of shock and trauma because we believe that the syndrome occurs only after severe shock and trauma and because this term by itself does not denote a specific aetiology. The RDS of shock and trauma should be distinguished from other causes of ventilatory failure that can follow shock and trauma. These other entities include pulmonary contusion, aspiration, atelectasis, pulmonary oedema secondary to congestive heart failure, pneumonia and embolism by large thrombi or fat. The differential diagnosis of these conditions will be considered in Section VI.
