Hyperlipidemia and proteinuria are frequently linked. The most notable clinical condition including these two factors is the nephrotic syndrome in which higher levels of proteinuria, elevated serum total cholesterol levels and reduced high-density lipoprotein (HDL) cholesterol represent three of the major signs of the disease. The reduction of normal proteinuria is followed by a normalization of the abnormal lipid profile. Hyperlipidemia and increased urinary albumin excretion are also frequently linked in patients with essential hypertension. This has raised questions regarding the cause-effect relationship between these two parameters. Moreover, no mechanistic explanation has been provided for this association. The evidence from patients with nephrotic syndrome would suggest that hyperlipidemia may be a consequence of pro-teinuria. More recently, however, some authors have suggested that hyperlipide-mia may play an active role in the progression of renal diseases. This raises the possibility that hyperlipidemia may be responsible for the renal damage and the attendant microalbuminuria or proteinuria in patients with essential hypertension. The purpose of this review is to discuss the evidence and the significance of the relationship between microalbuminuria and hyperlipidemia in patients with essential hypertension.
