The relationship between alterations of cerebrospinal fluid (CSF) fractions and disease states has been the subject of numerous investigations. Many studies have involved separation of proteins by electrophoresis'-5 but unfortunately this technic lacks sensitivity and does not measure individual proteins. The more useful electrophoretic patterns show an elevation in the relative proportion of gamma globulin (Ig), typically seen in multiple sclerosis, subacute sclerosing panencephalitis (SSPE) and inflammatory diseases of the nervous system. Using the more specific technic of radial immunodiffusion, Link and Mullere recently reported levels o f IgG, IgA and IgM in cases of multiple sclerosis and infections of the nervous system. Other workers',X also have used radial immunodiffusion for measuring C S F i in in u ti ogl o bu I ins but this tech n ic I ac k s precision and does not possess sufficient sensitivity without prior concentration of the sample to detect IgA in normal CSF. Electroimmunodiffusion"l 5 offers an advantage over the previously mentioned methods i n that the analysis can be carried out directly on unconcentrated spinal fluid. A recent report by Schuller'" describes an electroimmunodiffusion technic where the limits of sensitivity of IgA and IgM have been extended t o 0.10 and 0.13 mg per 100 ml, respectively. A relatively simple procedure was used recently by Laffin'" who measured IgG directly on unconcentrated C S F using the technic of simple immunodiffusion in narrowbore glass tubes as described originally by Oudin': in 1946 and more recently by Oudin" and Preer.Iy This very sensitive procedure, which requires as little as 50 pl of specimen for each determination, has been adapted in our laboratory2" to the measurement of IgG, IgA and IgM in unconcentrated CSF. The procedure has been found to correlate closely with immunochemical technics involving light scattering of antigen-antibody complexes.""" The present study was undertaken to evaluate the clinical usefulness of the immunodiffusion technic of Oudin for studying levels of IgG, IgA and IgM in CSF.
