We read with interest the article by Kasanuki and colleagues reporting a patient who presented with late-onset mania speculated to be associated with the Lewy body disease (LBD) pathology. 1 Although their case report lacked neuropathological confirmation, clinical presentation (ie, hyposmia and severe constipation) and neuroimaging studies were suggestive of the underlying pathology of LBD." Secondary mania" results from several causes, such as adverse effects of medication and neurological disorders, and is common in elderly patients. 3 Two patients developed mania simultaneously (Case 1) or a few years before (Case 2) the onset of parkinsonism or dementia.
