In this commentary, we discuss the challenges and opportunities for epidemiologic studies in evaluating breast cancer as a set of discrete diseases. We show examples of the strengths of the case-only design in assessing the relative correlation of established risk factors and the different cancer subtypes. We argue for the use of the case-only study design as an important initial step in understanding the extent of etiologic heterogeneity between tumor subtypes.
