The morphological and histochemical characteristics of the "hyaline droplets" which fill the proximal convolutions of the nephrons of individuals who, dying of hemorrhagic fever, had received large therapeutic infusions of concentrated human serum albumin have been compared with those of the droplets that form in experimental animals after the parenteral administration of proteins as well as with the droplets found in other renal diseases; the conclusion is drawn that all are protein absorption droplets. Experimental evidence is described which indicates that protein droplet formation is the pathological aspect of a physiological process, the continuous tubular reabsorption of plasma protein which normally filters through the glomerular membrane; droplets form whenever the disposal of the reabsorbed protein by the renal cells is at a rate insufficient to prevent intra-cellular accumulation. The mitochondria of the renal cells are directly involved in both the physiological and the abnormal reabsorption and disposal of the plasma proteins; the absorption droplets are a complex of reabsorbed proteins and mitochondrial substances and enzymes. Failure of intracellular disposal of reabsorbed protein is determined by (a) the quantitative and qualitative characteristics of the protein and (b) by the functional state of renal cell. Various factors in renal disease that result in disturbances of reabsorption and of intracellular disposal, both with and without droplet formation, are described.
