Fallon M, O'Neill W; eds. 68 pages. London: BMJ; 1998. $27.00. ISBN 072790793X. Order phone 44-171-387-4499. Field of medicine: Palliative care. Format: Softcover book. Audience: Physicians, nurses, pharmacists, and students seeking a general overview of pertinent issues in palliative care. Purpose: To provide a concise overview of the practice of palliative care. Content: This short book covers both the prevalent symptom control issues in palliative care and the nonphysical aspects of whole person care, which is characteristic of the field. It begins with a brief introduction to the principles of palliative care, followed by colorful, well-written chapters on the broad array of symptoms (such as difficult pain, nausea and vomiting, depression, dyspnea, anorexia and cachexia, and mouth and skin conditions) as well as chapters on special considerations in the care of persons with HIV disease or AIDS and care of children. Discussions of each symptom include information on diagnosis, pathophysiology, current standards of treatment, and areas of ongoing and future research. Other chapters emphasize the importance of such psychosocial issues as communication skills, attention to caregivers, community, and bereavement in the care of suffering patients and their caregivers. Highlights: Of particular note is the elegant overview of pertinent issues in palliative care of patients with HIV disease or AIDS, which emphasizes the importance of interdisciplinary care and sensitivity to potentially challenging social and cultural issues for both patients and caregivers. Also noteworthy is the chapter that provides practical management guidelines for the unique issues confronted in the last 48 hours of life. The chapter on issues in the care of children recognizes the importance of child development and the enhanced need to provide support for the family. Abundant photographs, illustrations, tables, and figures are included. Limitations: There are few references to direct readers to the next level of inquiry. Related reading: Story's Primer of Palliative Care (Univ of Texas, 1996) is a succinct booklet but lacks effective tables, color pictures, and figures. For those desiring more detailed discussion of a symptom or issue in palliative care, the Oxford Textbook of Palliative Medicine (Oxford Univ Pr, 1998), edited by Doyle, Hanks, and MacDonald, is encyclopedic and provides an exhaustive reference list.
