During a 12-month period (June 1985-May 1986) 17 cases of systemicHaemophilus influenzaeinfection (meningitis 7, bacteraemia 6, pneumonia 4) occurred in an aboriginal population of about 11 000 in central Australia, of whom 1540 were children <5 years of age. All these infections were in children <2 years of age. These figures are equivalent to an attack rate of 1100/100 000 children <5 years of age, which is exceptionally high. During the same period 7 cases ofH. influenzaeinvasive disease were detected in non-aboriginal children in central Australia (attack rate 350/100 000 children <5 years) which is also high. Data from the Adelaide Children's Hospital show that the rate in southern South Australia (which contains about 95% of the state's population) is about 30/100 000 children <5 years of age.The cases in aboriginal children occurred over a large and sparsely populated area. It is unlikely therefore that the high incidence rate is the result of a localized epidemic. Thus regions with climates as different as those of central Australia and Alaska share a high incidence ofH. influenzaedisease, which may be related to crowding and other adverse socioeconomic factors.newline˜M.P.E.
