To the Editor.— A few cases of mental confusion with cimetidine have been recognized. This has been seen after intake of a double or higher dose or at normal doses in older people. I wish to draw attention to visual hallucinations as a possible adverse effect of cimetidine therapy. Report of a Case.— A 72-year-old man was admitted with confusion, headache, and vomiting. He had sustained a head injury six weeks previously, for which he was given dexamethasone. He was found to be hypertensive and was treated with methyldopa and chlorothiazide. A CT scan and bilateral corotid angiograms disclosed bilateral frontoparietal chronic subdural hematomas. On the fourth hospital day a bilateral craniotomy was done, and liquified hematomas were evacuated. Postoperatively the patient was given 4 mg of dexamethasone every six hours, 30 cc of magnesium hydroxide (Mylanta) four times a day, and 100 mg of phenytoin sodium three times a
