Three siblings had Fe deficiency anemia without evidence of reduced Fe intake or gastrointestinal blood loss. They failed to respond to oral Fe therapy, and malabsorption of oral medicinal Fe was demonstrated in 2 of the children. All 3 had a partial but incomplete hematologic response to i.m. Fe dextran treatment. There was no evidence for other well-defined causes of hypochromic microcytic anemia or for a generalized disorder of intestinal absorption. These 3 patients appear to have a familial disorder characterized by impaired Fe absorption and utilization, similar to that observed in the mk/mk mouse.
