AIDS patients frequently develop pneumonia caused by Pneumocystis carinii. It has been conmonly believed that P carinii pneumonia (PCP) is caused by reactivation of latent organisms. However, it is not known when or how AIDS patients become infected with P carinii, and recent progress in understanding the nature of P carinii and its relationship with its host has called the role of latency into question. This review summarizes recent molecular genetic data on the nature of P carinii, reprises reports suggestive of interpersonal transmission of the microbe, and describes new information pertaining to the maintenance of P carinii in hunnans and laboratory aninnals. Progress in the application of 111olecular approaches to understanding the source of P carinii in AllXS patients is discussed.
