Allergic rhinitis (AR) might not appear to be a serious disease since it is not associated with severe morbidity and mortality, but, the symptoms can significantly impair the quality of life (QOL) of the patients. In Japan, Japanese cedar pollen (JCP) and house dust mites are common allergens, 1 and the number of AR patients has been increasing. 2 The guidelines for AR provide algorithms for developing treatment plans that depend on the severity of the allergic symptoms. 1 The goals of treatment are to reduce symptoms and to improve the QOL of the patients. Despite guideline-directed pharmacotherapy, 10% e20% of severe AR patients remain bothered by their symptoms. 3 Do such cases represent an “intractable” form of AR? Currently, there are no criteria for the determination of intractable AR. In the present study, we attempted to determine the characteristics of patients with intractable perennial AR (PAR) or seasonal AR (SAR). We administered questionnaires to clinicians with items considered to be potential clinical features of intractable PAR caused by mites or SAR due to JCP (SAR-JCP). There were ten items on the questionnaire: each item was carefully discussed and chosen by all of the authors (Table 1, 2). The clinicians were asked to choose three items that they considered to be the most important clinical features of intractable PAR caused by mites or SAR-JCP with unsatisfactory nasal symptoms despite guideline-directed pharmacotherapy, and to rank the three items according to importance. We assigned scores as follows to identify the items with the most importance as clinical features of intractable AR: the most important item (1st place) was assigned 3 points; the second most important item (2nd place) was assigned 2 points; and the third most important item (3rd place) was assigned 1 point. The scores for each item were summed (shown as the total score), and the average score among the clinicians was calculated to identify the rank of each item. The survey was conducted between June and August 2018.The questionnaire was distributed to a total of 5800 clinicians; these included 1846 otorhinolaryngologists belonging to the Japanese Rhinologic Society and 3954 allergy specialists belonging to the Japanese Society of Allergology, including physicians, ophthalmologists, dermatologists, pediatricians, and basic scientists who have a medical license.
