DAMAGE to the wall of the oesophagus by foreign bodies or instrumental trauma may involve only the mucosa, leaving the muscle layer intact. When the mucosal breach is complete, infected material from the mouth may gain access to the loose submucosa. Occasionally this proceeds further, and a dissection occurs with extensive separation of the mucosa from the oesophageal muscle. The dissection may extend pass the throat. Two nights before admission, however, she swallowed three aspirin tablets which blocked her throat and caused complete oesophageal obstruction. PHYSICAL EXAMINATION revealed no abnormality. INVESTIGATIONS.-on I May oesophagoscopy under general anaesthesia, and using a short Negus oesophagoscope, revealed a firm fibrous oesophageal web at the level of the cricopharyngeal sphincter.
