Integrated PET/MR is an imaging technique that combines the strengths of its constituent modalities and is uniquely suited for evaluation of skeletal metastatic disease. MR can contribute high soft tissue contrast and multiparametric anatomic imaging and has become the imaging modality of choice for assessment of musculoskeletal malignancies. PET provides quantitative molecular and physiologic information and has become a critical tool in the diagnosis and staging of malignancies. In this article, we will provide a brief overview of technical considerations for simultaneous PET/MR acquisition and their relevance to the detection and characterization of osseous metastases. We will also discuss principles for PET/MR protocol development that can optimize detection of skeletal metastatic lesions with a focus on technique for both 18F-FDG and non 18F-FDG radiopharmaceuticals. Lastly, we will review current evidence and provide recommendations for the use of PET/MR in patients for whom there is suspicion for skeletal metastatic disease.
