The application of a new ultrasonic velocity detector in the evaluation of peripheral vascular diseases is described. This instrument employs a transducer which may be used externally to assess qualitatively the velocity of arterial and venous flow. A bean of ultrasound is passed through the skin, subcutaneous tissue, and blood vessel. The particulate elements in blood shift the frequency of the transmitted signal by an amount directly proportional to their velocity. The shifted frequencies are amplified and presented as an audible signal. Since the instrument is applicable to the study of flow velocity at all levels of the arm and leg, it is possible to estimate the length of an occlusion and evaluate the patency of the vessels distal to the occlusion. With venous disease, the velocity detector has been found useful in localizing venous obstruction of the arm and In the leg from the level of the external lilac to the popliteal vein. Immediate and long-term assessment of the results of iliofemoral thrombectomy is also feasible. The dynamics of venous flow before and after ligation of the inferior vena cava may also be estimated.
