Multicentric Castleman’s disease (MCD) is a distinct type of lymphoproliferative disorder associated with human herpes virus-8 (HHV-8) and Kaposi’s sarcoma in the context of HIV infection with severely impaired cellular immunity [1,2]. Although MCD frequently runs a fatal course, early treatment, including highly active antiretroviral therapy (HAART), may prolong survival [3]. We describe rapidly fatal MCD in a patient with a failure to clear a high serum viral load of HHV-8 for more than 2 years, despite sustained full suppression of HIV upon HAART.
