Parotid saliva secretion rate and electrolyte composition were determined in thirty-one healthy children and twenty-nine children with cystic fibrosis. Flavored chicle was used as the stimulatory agent, and the un-contaminated parotid fluid was collected by means of a vacuum cup. The resultant data indicated that there were no significant differences in the secretion rate, pH, potassium and bicarbonate values between the groups. Although higher sodium and chloride concentrations were noted in the saliva obtained from the fibrocystic patients, the difference between the patient and control groups was only of "borderline" significance. Calcium and phosphorus levels were significantly elevated in the fibrocystic group. After subdivision of the control and fibrocystic groups (using the median secretion rate to divide each group) there was a statistically significant difference in the salivary sodium concentration between normal and fibrocystic children at low rates of secretion. At both low and high flow rates the salivary calcium concentration of the children with cystic fibrosis was significantly higher than the levels observed for the normal group. There was a significant difference in phosphorus concentration between normal and fibrocystic patients only in the subgroups above the median secretion rate. Information obtained in the current study seems to indicate that a defect exists in the metabolism of the parotid gland duct cells in children with cystic fibrosis. Presumably the duct cells fail to reabsorb sodium to the extent observed in normal children and secrete excessive amounts of calcium and phosphorus.
