Many in vivo platelet responses are considered to be mediated, directly or indirectly, by activated components of the complement system. These include the secretion of proteolytic enzymes and the assembly of key enzymes of the coagulation and fibrinolytic pathways. In this review, Peter J. Sims and Therese Wiedmer summarize the known interactions of human platelets with the complement system and discuss the implications of these interactions for platelet hemostatic function within the vasculature.It has been recognized for many years that immunological reactions can profoundly alter platelet function and survival within the vasculature 1-s. Some of these effects appear to be mediated directly by immunoglobulin or entail specific interaction of platelets with components of the complement system, while others are mediated through the interaction of platelets with inflammatory mediators released by immune-stimulated leukocytes. This review focuses exclusively on the response of platelets to direct interaction with one or more activated complement proteins. The effects of activated complement components on platelet hemostatic function will be assessed, as will the contribution of complementevoked platelet activation in various coagulopathies.
