Diabetes is the commonest cause of end-stage renal failure (ESRF) requiring renal replacement in the western world. It is also associated with a greatly increased incidence of cardiovascular disease with significant health and economic consequences. Cigarette smoking remains globally popular despite its well-publicised adverse effects. Recent interest has focussed on the role of cigarette smoking in the development and progression of diabetic kidney disease (DKD).
