The management of dementia with Lewy bodies (DLB) is a complex challenge for patients and caregivers. The disease is marked by fluctuating cognitive status with periods of lethargy, confusion, and inattentiveness. DLB is frequently complicated by disturbing delusions and visual hallucinations, as well as the motor syndrome of parkinsonism. As in idiopathic Parkinson’s disease (PD), parkinsonism in DLB is caused by the degeneration of dopamine neurons in the substantia nigra, which depletes the motor striatum of dopamine. PD and DLB share the pathological hallmark of Lewy bodies, proteinaceous inclusions containing the protein alpha-synuclein, suggesting pathobiological overlap. Regardless, dopamine replacement—such as levodopa—is a key tool for symptomatic control of parkinsonism in both diseases. Compared to that of PD, the parkinsonism of DLB can be less responsive to levodopa and its efficacy wanes more quickly than in PD (1, 2).
