Patients with pulmonary vascular disease invariably have an elevated pulmonary vascular resistance. The more severe and extensive the disease process, the greater this elevation in resistance. Unfortunately, these patients experience symptoms of exertional fatigue and breathlessness and signs of right heart failure only after their disease is far advanced. Methods of early detection are virtually nonexistent and clinical or noninvasive estimates of severity are often unreliable.Because the high resistance to flow in the diseased pulmonary circulation will adversely alter the cardiac output response to exercise and thereby compromise the aerobic capacity of the patient, exercise testing may prove useful in detecting and assessing the severity of pulmonary vascular disease. We have investigated the usefulness of exercise testing in patients with pulmonary vascular disease of varying severity and origin. Hemodynamic monitoring was combined with the measurement of respiratory gas exchange to examine these issues (1). In this report we review this experience, which indicates that the severity of pulmonary vascular disease can be determined through the noninvasive assessment of the maximal 02 uptake and anaerobic threshold attained during progressive upright treadmill exercise.
