Prophylactic measures, aimed at lifelong preservation of skeletal integrity should preferably be initiated in early adulthood, before the onset of significant bone decrements. Early intervention would tend to preserve the PSM of young adulthood, and thus provide a wider margin of protection for later years, when aging decrements and increased probabilities of confinement to bed may deleteriously affect skeletal balances. Clearly, prophylaxis initiated early in adulthood in women could not rely on estrogen replacement therapy, which is used prophylactically in postmenopausal years, because the premenopausal organism is estrogen-replete. Therefore, early strategy will have to rely on modifications of life-style elements.
