Sulfonylurea medications may have a beneficiary impact on poststroke functional outcomes with differences hinging on timing of medication exposure in relationship to acute ischemic stroke. 1, 2 We aimed to understand the association of sulfonylurea exposure on acute ischemic stroke functional outcomes. We hypothesized that sulfonylurea exposure would more likely associate with a favorable functional outcome.We performed a post hoc analysis of type II diabetics prospectively enrolled in the SHINE trial (Stroke Hyperglycemia Insulin Network Effort). 3 Detailed methodological techniques are available in the Supplemental Material. A total of 852 participants were included (Figure S1). Demographics are summarized in Table S1. In a 4-group pairwise comparison including both prestroke and poststroke exposure information, patients who had only prestroke exposure had a lower rate of favorable outcome, compared with those with no exposure or those with only poststroke exposure (Figur e S2). Patients with only prestroke exposure had significantly higher 90-day modified Rankin Scale score compared with those without any exposure (common odds ratio, 2.11 [95% CI, 1.43–3.12]; P< 0.001), while other groups had no significant differences (P> 0.05; Figure). Prestroke exposure was less likely to have favorable outcomes, and this association remained in those with nonlacunar strokes (adjusted odds ratio, 0.55 [95% CI, 0.35–0.85]; P= 0.007) but not in lacunar
