B型肝炎病毒(HBV)相关慢性肝病的发病机制仍不完全清楚。细胞因子谱的免疫失衡对HBV感染的消退和HBV清除产生深远影响。为探讨慢性乙型肝炎活动期患者外周血辅助性T细胞(Th)17和Th 1的免疫状态,对30例慢性活动期B型肝炎患者进行了研究。采用流式细胞术检测慢性B型肝炎(CH B)患者外周血Th 17细胞(CD 3(+)CD 8(-)IL-17(+)T细胞)、Th 1细胞(CD 3(+)CD 8(+)IFN-γ(+)T细胞)和Tc 1细胞(CD 3(+)CD 8(+)IFN-γ(+)T细胞)的比例。白细胞介素-17(IL-17)和干扰素-γ(IFN-γ)的蛋白和mRNA水平采用酶联免疫吸附试验(ELISA)和实时荧光定量聚合酶链反应(PCR)检测CHB患者外周血中Th 17细胞的含量(1.53 ± 0.52%)显著高于正常对照组(0.92 ± 0.20%)(P < 0.05)。与对照组相比,慢性乙型肝炎患者外周血Th 1、Tc 1细胞比例明显降低。慢性乙型肝炎患者外周血Th 17细胞与Th 1细胞呈负相关,与血清丙氨酸氨基转移酶呈正相关;慢性活动性B肝炎患者外周血Th 17细胞水平升高,提示其在慢性HBV感染的免疫激活中可能发挥作用。
The pathogenesis of hepatitis B virus (HBV)-associated chronic liver disease is still not fully understood. The immune imbalance of cytokine profile exerts a profound influence on the resolution of HBV infections and HBV clearance. This present study aimed to evaluate the immune status of the peripheral T helper (Th) 17 and Th1 cells in the active patients with chronic HBV infection.Thirty patients with chronic active hepatitis B were included in our present study. The frequency of peripheral Th 17 cells (CD3(+)CD8(-)IL-17(+) T cells), Th1 cells (CD3(+)CD8(-)IFN-gamma(+) T cells), and Tc1 cells (CD3(+)CD8(+)IFN-gamma(+) T cells) in chronic hepatitis B (CHB) were analyzed by flow cytometry. The protein and mRNA levels of interleukin-17 (IL-17) and interferon-gamma (IFN-gamma) were measured by enzyme-linked immunosorbent assay and quantitative real-time polymerase chain reaction (PCR).The percentage of Th17 cells in peripheral blood of CHB patients (1.53 +/- 0.52%) was significantly increased than that in normal controls (0.92 +/- 0.20%; P < 0.05). In contrast, the percentage of Th1 and Tc1 cells of CHB patients was significantly decreased as compared with that of control group. The frequency of Th17 cells had a negative correlation with Th1 cells, and a positive correlation with serum alanine aminotransferase in CHB patients.The elevated peripheral Th17 cells were obtained in the patient with chronic active hepatitis B, suggesting its potential role in the immune activation of chronic HBV infection.
