Mediators and mechanisms associated with paroxysm in Plasmodium vivax malaria

The symptoms of the paroxysms ofPlasmodium vivaxmalaria are described, followed by a summary of what is known about the cause of the symptoms and a discussion of the biological significance of the paroxysm; several experiments, some already reported in previous publications, are briefly described. Based on these results, the authors conclude that the plasma of paroxysmal patients (but not that taken from patients shortly before or after a paroxysm) inactivatesP. vivaxgametocytes, and that this effect is not solely due to temperature. Monocytes, tumour necrosis factor α and parasite products are all necessary but not sufficient for this inactivation. The effect is mediated by nitric oxide (NO). Interleukin-2 and granulocyte-macrophage-colony stimulating factor are both necessary. The parasite products are antigenically distinct and specific to the parasite species. The implications of these findings for the mechanism by which the paroxysm inactivates parasites are discussed; it is concluded that antigen-specific T cells and species-specific antigens are involved, and a scheme for the process is presented in a diagram.
