The article presents a discussion on inflammation as a cause of functional bowel disorders (FBD). Inflammation does appear to play an important role in FBD, so far the most investigated condition is irritable bowel syndrome (IBS). Mast cells have continued to gain attention as a possible cause of IBS symptoms because of their potent pro-inflammatory content that can alter both neural and smooth-muscle function. The observation that a great many patients develop persistent IBS symptoms after an acute bout of infectious gastroenteritis can be considered a prototype example of how immune-mediated mechanisms play a role in the pathogenesis of IBS.
